What it is

The special healthcare agreement (convenio especial, CE) lets people who live in the Community of Madrid and are not recognised as entitled to healthcare from the National Health System receive care from the public system. Requirements: living in the Community of Madrid, proving at least one continuous year of actual residence in Spain or in another EU or EEA country, Switzerland or the United Kingdom immediately before applying, being registered on the padrón in a municipality of Madrid and paying a monthly fee (60 euros if under 65 and 157 euros if 65 or over). Each family member files their own application, online or in person, with their full passport and historical padrón record. The decision comes within a month, after which you must go and sign the agreement. No health card is issued: you receive a Documento para Asistencia Sanitaria. It includes the basic common services of the National Health System, but not outpatient pharmacy, orthoprosthetic products, dietary products or non-emergency medical transport. If two consecutive fees are not paid, the agreement ends and cannot be signed again until a year has passed.

When

For people living in the Comunidad de Madrid who are not entitled to National Health System care. You must be registered in the Comunidad de Madrid and prove one continuous year of actual residence in Spain or another EU/EEA country, Switzerland or the UK. You can apply at any time.

Steps

  1. Step 1: Submit the application

    Online with an electronic signature, or by downloading the completed form and submitting it signed at a registry office. One application per family member.

  2. Step 2: Wait for the decision

    A decision is issued within a month. If something is missing, you are asked to correct it within 15 days; otherwise it is closed as withdrawn.

  3. Step 3: Sign the agreement and pay monthly

    You go to sign on the appointment date (you have up to three months from notification). You get a copy, instructions for paying the fees and the “Documento para Asistencia Sanitaria”, which you must show at health centres.

Do I get a health card?

No. The agreement does not include an individual health card; you are given a personal ID code and the Documento para Asistencia Sanitaria.

What happens if I stop paying?

It ends if you do not pay the first fee, two consecutive months or three non-consecutive months. You then cannot sign another until a year has passed.

Is it valid outside the Comunidad de Madrid?

If you travel temporarily to another region, you get the same services there.
