What it is
The Conselleria assigns your health centre according to your padrón address, but anyone with a SIP card who is insured or a beneficiary of the National Health System can ask to choose another family doctor, paediatrician or primary care centre. The holder, an authorised person or a legal representative (for example, of a minor) applies. The request is decided within 15 working days at most, after a report from the chosen professional; it can be refused, for example, if that professional already has more patients assigned than allowed. If you choose a doctor at a centre other than the assigned one, home visits are still provided by the original centre. With a digital certificate or Cl@ve it can be done online.

Steps
Step 1: Submit the application
Go to the official website: Free choice in primary care (GVA)The application is addressed to the head of the basic health area of the chosen centre. It can be submitted on the e-office with a digital certificate, DNIe or Cl@ve permanente. The holder, an authorised person or the legal representative can apply.

Step 2: Wait for the answer
The chosen professional issues a report and a decision is made within 15 working days at most. If accepted, your assignment is updated in the SIP; if refused, you are told in writing with the reasons.
