Choosing a family doctor and CAP (Catalonia)

What it is
In Catalonia, to change your family doctor, paediatrician or nurse at the CAP you follow the same procedure as for choosing another primary care centre: you ask for the application form (or download it from the website) and take it completed to the new centre, or to your own if the change is within the same CAP. If the centre belongs to the Institut Català de la Salut (ICS), you can also do it online or in La Meva Salut. The chosen professionals must belong to the same primary care team. If you are not answered on the spot, you are informed within ten working days at most; if there is no answer after that period, the request is deemed accepted.

Step 1: Get and fill in the form
Ask for the form at the CAP or download it from the CatSalut website and fill it in.

Step 2: Hand it in at the CAP
Go to the official website: Free choice procedure (CatSalut)Take it to the new centre where you want to be seen, or to your own if the change is within the same CAP. If your centre belongs to the ICS, you can also do it online or in La Meva Salut.

When will I get an answer?
